Welcome to the Farabaugh Chiropractic Office and thank you for choosing to schedule your appointments using our online appointment system.
We are very excited to be able to offer this service to you. We realize how hectic and chaotic life has become and how any convenient time saver that we can offer will be appreciated. With our new system you will no longer be bound by the hours of typical operation since internet access is a 24 hours per day, 7 days per week service. As we continue to develop online scheduling, you'll find it easier and easier to schedule an appointment.

1) Simply call the office and leave a message: 614-898-0787.
2) Email us at chironf@aol.com and indicate which doctor you would like to see, which day, and which time.
3) Complete the form below, print this page, and then fax it to the office at 614-898-1945. We will call/contact
you to confirm your appointment.

To save you time, you can also download the new patient forms (See the New Patient Forms link on the home page) to complete at your convenience prior to consultaing our office. That simple step saves you approximately 20 minutes during your first visit. If you are a new patient, please allow approximately 30-60 minutes for your first appointment.
Cancellation Policy: Should you need to cancel an appointment, we would appreciate 24 hours notice when possible. We do understand that emergencies arise and we will accept your cancellation with later notice. However, if it is one hour or less before your appointment please call us at 614-898-0787, so that we may notify your doctor or clinical assistant.
